Juvier Family Insurance LLC

Independent Insurance Agency — Licensed in Florida, Texas, and New Mexico

Effective Date: 01/15/2026

1. Introduction

Juvier Family Insurance LLC ("Juvier Family Insurance," "we," "us," or "our") is an independent insurance agency that helps individuals and businesses find, compare, and purchase insurance coverage through relationships with numerous insurance carriers across the United States. We respect your privacy and are committed to protecting the personal and financial information you share with us.

This Privacy Policy explains what information we collect, how we use and share it, the choices available to you, and the steps we take to safeguard it. It applies to information we collect through our website, by phone, by email, in person, and through any other means in the course of providing insurance quoting, placement, and servicing.

This policy is provided as general guidance and is not a substitute for legal advice. Insurance privacy obligations vary by state and by line of business; we recommend having this policy reviewed by a licensed attorney familiar with insurance regulation in Florida, Texas, and New Mexico before publishing it.

2. Who We Are and Scope of This Policy

Juvier Family Insurance LLC is an independently owned and operated insurance agency. We are not an insurance carrier — we act as an intermediary, representing our clients in shopping for and servicing insurance policies issued by third-party insurance carriers, including most major carriers licensed to do business in the United States.

We are licensed to transact insurance business in the following states:

  • Florida
  • Texas
  • New Mexico

This Privacy Policy applies to prospective, current, and former clients, as well as visitors to our website, regardless of the state in which they reside, unless a more specific state or federal law requires additional or different disclosures — in which case that law controls for residents of that state.

3. Information We Collect

Depending on the products or services you request, we may collect the following categories of information:

3.1 Personal Identifying Information

  • Full name, date of birth, gender, and marital status
  • Mailing address, email address, and phone number
  • Social Security number or Tax Identification Number (as required by carriers for underwriting)
  • Driver's license number and driving history

3.2 Financial and Insurance Information ("Nonpublic Personal Information")

  • Income, assets, and credit-based insurance scores
  • Bank account or payment card information for premium payments
  • Current and prior insurance policy details, claims history, and loss history (e.g., CLUE reports)
  • Property information (address, value, construction details) for property and casualty coverage
  • Vehicle information (VIN, make, model, usage) for auto coverage

3.3 Health Information (Where Applicable)

For life, health, disability, or long-term care insurance quotes, we may collect limited health-related information necessary for underwriting, such as medical history, height/weight, tobacco use, and prescription history. This information is treated as sensitive and is shared only with the applicable carrier(s) or as otherwise permitted or required by law.

3.4 Business Information

For commercial lines clients, we may collect business name, federal EIN, entity type, industry classification (NAICS/SIC code), payroll and revenue figures, property schedules, and employee information necessary for commercial underwriting.

3.5 Website and Technical Information

  • IP address, browser type, device type, and operating system
  • Pages visited, referring URLs, and time spent on our site
  • Information submitted through online quote forms, contact forms, or chat tools

4. How We Collect Information

We collect information directly from you when you request a quote, complete an application, communicate with us by phone, email, or online form, or otherwise interact with our agency. We also collect information from the following sources:

  • Insurance carriers and third-party administrators we work with to place or service your policy.
  • Consumer reporting agencies, for items such as credit-based insurance scores, motor vehicle records (MVR), and claims history reports (e.g., CLUE, A-PLUS).
  • Publicly available sources, such as public property records.
  • Affiliates or referral partners, if you were referred to us.
  • Automated technologies on our website, such as cookies and similar tracking tools (see Section 9).

5. How We Use Your Information

We use the information we collect to:

  • Provide insurance quotes and compare coverage options across carriers
  • Prepare, submit, and process insurance applications on your behalf
  • Service existing policies, including endorsements, renewals, and claims assistance
  • Verify your identity and prevent fraud
  • Communicate with you about your policies, quotes, billing, and renewals
  • Comply with legal, regulatory, and licensing obligations
  • Improve our website, services, and customer experience
  • With your consent, send marketing communications about products that may interest you

6. How We Share Your Information

As an independent agency, sharing certain information with insurance carriers is necessary to obtain quotes and place coverage. We share information with:

  • Insurance carriers and underwriters to obtain quotes, bind coverage, and service policies.
  • Consumer reporting agencies and vendors that support underwriting (e.g., MVR providers, loss-history databases, credit bureaus).
  • Service providers who perform functions on our behalf, such as agency management software, payment processing, email/communication platforms, and IT support — under contractual confidentiality obligations.
  • Regulators and law enforcement, when required by subpoena, court order, state insurance department inquiry, or other legal process.
  • Successors, in the event of a merger, acquisition, or sale of some or all of our agency's assets.

We do not sell your personal information to third parties. We do not share your nonpublic personal information with unaffiliated third parties for their own independent marketing purposes, except as permitted by law or with your consent.

7. GLBA Notice & Your Opt-Out Rights

As a licensed insurance producer, Juvier Family Insurance LLC is subject to the privacy provisions of the federal Gramm-Leach-Bliley Act (GLBA) and corresponding state insurance privacy regulations adopted by Florida, Texas, and New Mexico (each generally based on the NAIC Insurance Information and Privacy Protection Model Act and/or the NAIC Privacy of Consumer Financial and Health Information Model Regulation).

Under GLBA, we are permitted to share nonpublic personal information with nonaffiliated third parties as necessary to service your insurance needs (for example, sending your application to a carrier) without offering an opt-out, because these disclosures fall under statutory exceptions. For any sharing that falls outside those exceptions — such as sharing your information with an unaffiliated company for its own marketing purposes — we will provide you the opportunity to opt out before doing so.

To opt out of marketing communications, or to limit sharing of your information where legally required, contact us using the information in Section 15. We will process your request within a reasonable time and, where required by law, no later than the timeframe specified under applicable state insurance regulations.

8. State-Specific Privacy Rights (FL, TX, NM)

Insurance-specific privacy protections in the states where we are licensed generally derive from each state's insurance code and the NAIC model acts referenced above, in addition to GLBA. Depending on your state of residence, you may have rights that include:

We maintain administrative, technical, and physical safeguards designed to protect your personal and financial information from unauthorized access, disclosure, alteration, or destruction. These measures include restricted access to client records, secure handling of documents containing sensitive information, and use of reputable, security-conscious vendors and agency management systems. No method of transmission or storage is 100% secure, and we cannot guarantee absolute security.

11. Data Retention
